An organiser will submit a new application for a permit for an authorised childcare facility:
- when there is a change of organiser;
- when the childcare facility changes its location;
An organiser will submit an application for the adaptation of
the permit granted to a childcare facility when it wishes to increase
the number of places in the childcare facility.
Action to be taken
You must send a request with a detailed description to Iriscare.
Required documents
The application for a permit must be submitted to the Administration. The application must be accompanied by a detailed description which includes the following data:
1. data relating to the organiser:
(a) name, legal form, address and company number;
(b) the statutes, if the organiser is a legal person;
(c) a copy of the identity card, if the organiser is a natural person;
(d) the document referred to in Article 47, § 2;
(e) for any organiser with more than 10 childcare places, the organisational structure and evidence that at least one person has the necessary knowledge to ensure the organisational management of the childcare facility, as referred to in Article 44;
2. data relating to the childcare facility:
(a) name and address;
(b) the number of childcare places requested;
(c) confirmation that the facility is operated in a professional capacity and for a fee;
3. the data relating to the person responsible:
(a) the identity and contact details of the person responsible, at least their surname and first name, telephone number and email address;
(b) the documents referred to in Article 39, § 2;
4. the data of those persons accompanying the children:
(a) data relating to the identity of these persons;
(b) the documents referred to in Article 41, § 2;
5. data relating to other persons in the childcare facility who have direct contact with the children in their care:
(a) the identity of these persons and their function or capacity;
(b) the documents referred to in Article 43;
6. the plans for the infrastructure, including at least the number of rooms and the purpose and size of these rooms;
7. for childcare facilities with more than six places, the fire safety certificate as referred to in Article 9 § 1, dated no more than 6 months previously;
8. the risk analysis as referred to in Article 34;
9. a description of the crisis procedure as referred to in Article 35;
10. a description of the procedure for dealing with inappropriate behaviour as referred to in Article 36;
11. the insurance as referred to in Article 37;
12. a description of the procedure for dealing with complaints, as referred to in Article 54;
13. the quality manual as referred to in Article 55;
14. a description of the educational policy, as referred to in Article 57;
15. the internal rules as referred to in Article 64;
16. a draft written agreement, as referred to in Article 66.
