During the period of the provisional operating permit, officials will check that the service is operating in accordance with all the standards which it is required to meet.
The officials will draw up a report with regard to the application for accreditation at the latest six months before the end of the provisional operating permit, including any comments which they may have. The file and this report will be transmitted to the Ministers; the report will be transmitted simultaneously to the organising authority, which will have a period of one month to submit its observations with regard to the content of the report to the Ministers and the department.
Required documents:
The organising authority will submit an application for accreditation to the Ministers, together with an administrative file containing the following documents:
1. a document mentioning the names of the representatives of the organising authority and of the coordinator of the centre; it must be signed by the above-mentioned persons;
2. an updated copy of the statutes for the centre, drawn up in French and Dutch, as well as the composition of its management bodies, as published in the Belgian Official Gazette. Where the organising authority is a mutual society or union of mutual societies, or a public authority as referred to in Article 4 of the Ordinance, this is a copy of the decision of the body competent to set up the centre;
3. (a) in the case of an existing centre: the organisational chart and the list of persons employed as well as their qualifications and the actual working hours during the previous quarter;
(b) in the case of a centre which has been brought into operation for the first time: the organisational chart and the commitment to comply with the staffing standards and to send the Ministers a list of the persons employed, their qualifications and the weekly working hours every six months;
4. a plan showing the internal communication routes of the centre and, where applicable, its branches, and the purpose of the premises;
5. where appropriate, a copy of the agreement between the centre for which accreditation is sought and the institutions with which a functional link is to be provided in accordance with the accreditation standards in force;
6. a copy of the report with regard to the fire safety of the centre drawn up by the Fire and Emergency Medical Service of the Brussels-Capital Region.
This report may not be older than six months at the time when the application is made;
7. a certificate of good conduct from the centre's coordinator and staff, dated no more than one month before the application is submitted;
8. a document defining in particular:
(a) the goals pursued by the centre;
(b) the means used to achieve them;
(c) the tasks of the members of staff;
(d) the centre's identification questionnaire, issued for this purpose by the administration, duly completed and signed.completed and signed.
